Completion of HRA
$25 incentive reward in your OTC Mastercard for completing the Health RiskAssessment (HRA). You may complete this assessment by phone or in person with your Service Coordinator. A copy will also be sent to you in your welcome packet, you may complete it and return it by mail. Available once a year.
